How OsteoPerio handles website enquiries and personal data.

The website owners are Dr. Sahand Shirzadi Maryan and Dr. Sergi Rivas Frau, operating under the brand OsteoPerio. Both doctors have access to enquiries sent to the team’s mailbox. The responsibilities for handling enquiries and the full controller identification are being finalised. The confirmed ownership details and outstanding legal identification items are listed in the Legal Notice.

• Identification and contact data, such as name, email address, phone number, WhatsApp number, country, city, and preferred language.
• Appointment and enquiry details, such as preferred contact method, preferred visit month, and the type of request submitted.
• Clinical context voluntarily provided in the form, such as main concern, previous surgery, availability of X-rays or CBCT, and other health-related information you choose to include.
• Professional enquiry data, such as referring dentist or clinic details, and the qualifications and training interests of dentists contacting us about education.
• Technical data required for security, hosting, form delivery, and website operation.

When you submit the enquiry form, your browser sends the information to Web3Forms for email delivery to the OsteoPerio team. Please provide only what is needed for your initial enquiry. The form does not accept file uploads; our team can explain which records are needed and how to send them separately.

Form submission analytics records a submission status and a fixed technical error category when applicable. It excludes your name, contact details, clinical concern and message.

• To respond to appointment requests, international case review requests, and general enquiries.
• To coordinate communication with patients, referring doctors, students, or professional collaborators.
• To understand the general clinical concern before advising whether a formal consultation or additional records may be needed.
• To maintain website security, prevent abuse, and improve the user experience.
• To comply with legal, professional, clinical, and administrative obligations where applicable.

The website enquiry form asks for your consent to use the submitted information to review and respond to your request, including any health information you choose to provide. You can withdraw that consent by contacting the privacy email. Withdrawal does not affect processing already carried out lawfully.

A website enquiry is separate from clinical care. If you become a patient, the treating professionals must explain the arrangements for your clinical record and the applicable healthcare obligations. Consent to an enquiry is not consent to treatment or marketing.

The enquiry form sends the information you submit to Web3Forms for delivery to our team by email. The website uses Vercel hosting, Web Analytics and Speed Insights. Opening a map, WhatsApp or social link takes you to a separate provider.

Web Analytics measures visits and interactions such as enquiry-button clicks. Speed Insights measures page loading and responsiveness. These services can process the page URL, referring website, browser and device information, approximate location and performance measurements.

The form submission events we send contain only a submission status or fixed technical error category; they do not include the form fields or message. See the Cookie Policy for browser storage and provider information.

Service providers may process information outside the European Economic Area. Web3Forms states that it operates from India and uses infrastructure in multiple regions; its privacy policy describes its processing and transfer safeguards. Contact our privacy email for information about the arrangements applicable to your enquiry.

A fixed team retention schedule for website enquiries has not yet been established. We are finalising criteria that distinguish enquiries that go no further, ongoing correspondence, and information that becomes part of clinical care or is needed for applicable administrative or legal obligations. Clinical documentation is separate from an initial website enquiry; the treating professionals explain the arrangements for your clinical record.

Provider retention is separate from the team’s email records. Web3Forms currently states that form submissions may be retained for up to three years, unless a shorter plan period or earlier deletion applies. Contact us to request information about retention or deletion of your enquiry.

International patients may contact OsteoPerio before travelling to Barcelona. Initial website contact does not replace diagnosis or create a confirmed treatment plan. Additional records, clinical examination, informed consent, and suitability assessment may be required before treatment is proposed.
